Osseous Surgery in Sterling, Leesburg & Aldie, VA

At NOVA Implant & Perio Specialists, Dr. Jean-Claude Kharmouche provides osseous surgery (periodontal pocket reduction surgery) for patients with advanced periodontal disease that cannot be controlled with deep cleaning alone.
Osseous surgery removes infected bacteria, smooths damaged bone, and reduces periodontal pockets—restoring healthier gum attachment and preventing further tooth loss.

Patients across Sterling, Leesburg, and Aldie, VA trust our team for surgical precision and long-term periodontal stability.

What Is Osseous Surgery?

Osseous surgery is a periodontal procedure performed to clean advanced infection below the gumline and reshape damaged bone around the teeth. When gum disease progresses, deep pockets form, allowing bacteria to grow where brushing and flossing cannot reach.
Osseous surgery:

  • Removes bacteria and tartar from deep pockets
  • Repairs and smooths irregular bone
  • Allows the gums to reattach more securely
  • Prevents ongoing bone and tooth loss

This treatment is often the final step in stopping periodontitis from progressing.

Benefits of Osseous Surgery

Significant reduction in pocket depth
Better access for home cleaning and hygiene
Prevention of future bone and gum destruction

Our Osseous Surgery Process

Step 1: Periodontal Evaluation
Dr. Kharmouche performs a detailed exam, measuring pocket depths and reviewing X-rays to evaluate bone loss and infection severity.
Step 2: Accessing Deep Pockets
Under local anesthesia (with sedation available), gum tissue is gently lifted to expose areas of infection and damaged bone.
Step 3: Cleaning & Bone Reshaping
Bacterial buildup and tartar are removed, and rough or uneven bone surfaces are smoothed to eliminate pockets and encourage healthy reattachment.
Step 4: Reattachment & Healing
The gum tissue is repositioned to fit snugly against the newly shaped bone, promoting stable healing and easier long-term maintenance.

Why Is Osseous Surgery Necessary?

Scaling and root planing is effective for early-stage gum disease, but advanced bone loss requires surgical treatment.

Osseous surgery may be recommended if you have:

  • Deep periodontal pockets
  • Continued bleeding or inflammation
  • Bone loss visible on X-ray
  • Loose or shifting teeth
  • Persistent infection after deep cleaning
  • Difficulty maintaining the area with home care

Without treatment, periodontal disease can continue to destroy bone and lead to tooth loss.

What to Expect After Osseous Surgery

Healing is typically smooth and well-tolerated. Most patients experience:

  • Mild soreness for a few days
  • Reduced inflammation within weeks
  • Significant improvement in gum stability
  • Ability to maintain better oral hygiene long-term

A follow-up periodontal maintenance schedule every 3–4 months ensures the treated areas remain healthy.

Osseous Surgery vs. Other Periodontal Treatments

Osseous surgery may be recommended instead of—or in combination with—other treatments:

Scaling & Root Planing:
Used first; osseous surgery is recommended if deep pockets persist.

Laser Therapy:
Effective in certain cases but may not reshape bone or reduce deep pockets fully.

Gum Grafting:
Used for recession; osseous surgery targets infection and deep pockets, not gum coverage.

Regenerative Procedures:
When possible, regeneration is used, but osseous surgery is needed when the bone cannot be rebuilt.

Dr. Kharmouche will recommend the most conservative and effective option based on your exam.

Frequently Asked Questions About Osseous Surgery

Is osseous surgery painful?
The procedure is performed under local anesthesia, and sedation is available. Post-op discomfort is typically mild and managed with over-the-counter medication.

How long does recovery take?
Initial healing takes 1–2 weeks, with full tissue maturation over several months. Most patients return to normal routines quickly.

Will my gums look different after surgery?
Some gum shrinkage is normal because pockets are reduced. This allows for easier cleaning and healthier long-term results.

Is osseous surgery covered by insurance?
Often yes—because it is medically necessary for controlling gum disease. Our team verifies benefits for all three of our Northern Virginia locations.
